Alex,
If you wait for SBS 2003 R2 Premium Edition, that's exactly what you will
get. It should be available soon, so they tell us.
http://www.microsoft.com/windowsserver2003/sbs/r2/default.mspx
If you purchase a new server now with SBS 2003 installed on it, you will get
a copy after it ships for a shipping charge only.

If you buy SBS 2003 at retail or the VL version, you'll have to pay for it.
($$$)
Your SBS CAL is all that's required. If you purchase SQL 2005 WE
standalone, then yes, you need a CAL for every device or user.
