In the Account list, I see I can edit any investment account and enter
what looks like a per transaction fixed fee. I have that situation at
Smith Barney, but after entering the $6 and a few transactions, I
don't see this fee entering into the transaction anywhere. There is no
Misc Exp transaction generated or any other record of it. Have I
misunderstood the whole process?

I don't know what that option is intended for ... but I have never known
it to have any affect on Quicken transactions.
