If you search in Quicken Help in their support center, you'll find an
entry that describes this as a known problem with Quicken 2007 R2. You
can sign up for notification of changes to their reply to this problem.

I am having the same problem.
Please report it.
The more people that do, the more chance the bug will be expedited for a
fix.

I've just discovered a workaround (well, kind of anyway). When I open
Q2007 R2 the "extra" icons are missing. If I then do a "backup" to
disk, after the backup the icons magically reappear!

I see what you mean - I backuped Quicken from the File Menu and the
icons I added reappeared, but once I close Quicken, the icon is gone
again. So the options are to either readd the icon, or immediately
backup Quicken as soon as you open the program. Still un-necessary
steps!
