If I understand you correctly, this should never happen. Any
account that is activated for downloading should appear in the
Pin Vault; it's activating an account that puts its name in the
Pin Vault (and deactivating should remove its name from the Pin
Vault).

I assume you are referring to entering the user-id and password
in the activate dialog.
There are no requirements that the Quicken user-id or password
be the user-id/password that you use to logon to your financial
institution web site; though I did think that you would use the
web site logon info for the Quicken account setup logon.
I'd double check with the fi.

You could try deActivating the account for downloading, then
reActivating it.
If you really had a Quicken account that was Activated for
downloading and the account name did not appear in the Pin
Vault, perhaps you have a corrupted Pin Vault. The solution for
that is to delete the Pin Vault and re-create it. Unless you
are certain you have memorized every password in the vault,
before deleting the Pin Vault, Print it; that will also print
the passwords.

I doubt this is the same problem you're having. There are a few
users who have been experiencing problems with wanting to enter
a password in the Pin Vault that is truncated while being
entered in the Pin Vault at a length less than their actual
password. But I think that problem has to do with Quicken not
having the most current info for editing that particular fi's
passwords.
The dialog for activating an account will accept something like
31 characters for the password.
