Has anyone ever used a 1099 issue service such as

or any other? Does anyone have a recommendation for a
reliable one?

Most CPAs will prepare 1099s for their clients.

I used www.eFileforBusiness.com last year to file two
1099's. The process was easy and worked, but I can't say
if they are any better or worse than other such services.

Do you recall if the $3.49/form charge requires some minimum
number of forms? I couldn't find it on the site under
products or services.

While I answered this privately via an e-mail request,
others may be interested. There is no minimum as far as I
know. I filed only two 1099's last year and the cost was
$3.49 each.
