Does anyone know if NY will accept a 7004 instead of the NY
Extension request? The instructions give the NY form to
file, but they don't say if it's mandatory. Needless to
say, NY is asking for a $300 penalty, but I don't think they
got the copy of the 7004.

I always file the NYS extension form - becuase you have to
make a payment (even if its the minimum tax), so I don't see
how sending a copy of a Federal form is going to help you.
